Ordering repeat prescriptions

As per NICE guidelines, to ensure that the correct medication and dosage is prescribed, ALL prescriptions must be requested in writing. You can do so by completing the following form, requesting through your Pharmacy or completing a paper prescription request form and handing into the Reception. Prescription requests are processed within three working days.

Please note, we are unable to accept prescription requests over the phone. Please submit request using the below form or via Patient Access. Alternatively, you can ask your Pharmacy to request on your behalf or you can hand in a request form at Reception. Prescription requests take up to three working days to be issued.

Charge Exemption Forms

You can apply for a Medical Exemption Certificate by completing an MEDEX application. These applications can be requested at our Reception.

This form can only be completed by a GP or medical representative who has access to your medical records.
