As per NICE guidelines, to ensure that the correct medication and dosage is prescribed, ALL prescriptions must be requested in writing. You can do so by completing the following form, requesting through your Pharmacy or completing a paper prescription request form and handing into the Reception. Prescription requests are processed within three working days.
Please note, we are unable to accept prescription requests over the phone. Please submit request using the below form or via Patient Access. Alternatively, you can ask your Pharmacy to request on your behalf or you can hand in a request form at Reception. Prescription requests take up to three working days to be issued.

The NHS charge a flat-rate amount for prescriptions, deemed appropriate by the Government for those who pay for their prescriptions. Patients who require extensive medications may benefit from Prescription Prepayment Certificates (PPCs). For more information, contact the PPC helpline on 0300 330 1341.
There are exemptions and remission arrangements in place to protect those who may need financial support for prescription charges. These apply to NHS prescriptions, dental charges, optical and hospital travel costs. There is further information about prescription exemptions and fees on the NHS website.
Antibiotics Resistance
Antibiotics do not fight viral infections. For most common viruses such as cold, flu and many sinus and ear infections, over the counter symptom relief is the best treatment option.
Using antibiotics appropriately is best for your health, your family’s health and the health of those around you. Misuse of antibiotics has contributed to antibiotic resistance. 25,000 people every year die each year from infections resistant to antibiotics.
If you are prescribed antibiotics, take them exactly as prescribed and always complete the course. They should not be saved for a later date or shared with others.
